Is there any way possible to set my Reviews app so that only admin staff can add custom categories? Allow members to post but not create custom categories?

You can pre define categories -- look in the admin options and you will see that section. You cannot however stop members from suggesting categories for this app. If you have not predefined a category that works and a member creates one, you can either allow it, or edit it under an existing category.

I did set up some pre-defined categories but did not see an option that would notify me when others are created. Is this automaitc for admins with the application?

In the event I need them, are sub-categories available? I didn't see this choice but want to make sure

There is no notification specifically for the creation of categories... but you can be notified when a member adds an entry. Then, you can check the category used. There are no subs for this app at this time.
