Is there any way possible to set my Reviews app so that only admin staff can add custom categories? Allow members to post but not create custom categories? ______________________

You can pre define categories -- look in the admin options and you will see that section. You cannot however stop members from suggesting categories for this app. If you have not predefined a category that works and a member creates one, you can either allow it, or edit it under an existing category. _________________________________

I did set up some pre-defined categories but did not see an option that would notify me when others are created. Is this automaitc for admins with the application?
In the event I need them, are sub-categories available? I didn't see this choice but want to make sure

There is no notification specifically for the creation of categories... but you can be notified when a member adds an entry. Then, you can check the category used. There are no subs for this app at this time. _________________________________
